Job summary

Embrace Hospice in San Antonio, TX seeks a licensed Practical Nurse (LPN/LVN) to provide compassionate end-of-life care in a home-focused hospice setting. You will follow patient care plans, document care accurately, and support families with education and guidance.

Requirements include a valid LPN/LVN license, CPR where required, and at least three years of nursing experience (hospice/medical-surgical preferred). A driver’s license and reliable vehicle are expected.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; hospice or medical/surgical preferred.
• Current LPN/LVN license in the state.
• Adherence to professional standards and practices.
• Valid driver's license and insured vehicle.
• CPR certification current where required.
• Understanding hospice philosophy and patient needs.
• Strong verbal and written communication and organizational skills.

Responsibilities

• Provide hands-on care to patients nearing end of life per state nursing rules.
• Follow the care plan after assessing patient needs.
• Keep clear and timely records of care provided.
• Educate and support patients and families about illness and care plan.
• Coordinate care by communicating with hospice team.
• Manage pain and symptoms and evaluate treatment effectiveness.
• Participate in training and quality improvement activities.

As a Hospice LPN You will:

• Give hands‑on care to patients nearing the end of life, following state nursing rules.

• Follow the care plan after assessing the patient’s needs.

• Keep clear and timely records of the care provided.

• Teach and support patients and their families about the illness and care plan.

• Help coordinate care by communicating with the hospice team (like aides, chaplains, social workers).

• Manage pain and symptoms, checking how well treatments are working.


• Take part in training and quality improvement activities to grow professionally and improve care.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.

• Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.

• Currently licensed as an LPN/LVN in the State.

• Comply with accepted professional standards and practice.

• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.


• Possess and maintain current CPR certification in states where required.

• Understand philosophy of hospice concept. Also understand needs of the terminally ill.

• Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.

• Demonstrate good verbal and written communication, and organization skills.
