Job summary

Embrace Hospice is seeking a licensed LPN/LVN in San Antonio, TX to provide compassionate end-of-life care in a home setting. You will follow individualized care plans, monitor patient comfort, and coordinate with a multi-disciplinary hospice team to ensure high-quality care for patients and families.

The role offers a supportive work environment, opportunities for professional growth, and a schedule that aligns with a Monday–Friday workweek.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; hospice or medical/surgical experience preferred.
• Currently licensed as an LPN/LVN in the State.
• Comply with accepted professional standards and practice.
• Must be a licensed driver with insured vehicle.
• Possess current CPR certification where required.
• Understand hospice philosophy and needs of the terminally ill.
• Demonstrate good verbal and written communication and organization skills.

• Keep clear and timely records of care provided.

As a Hospice LPN You will:

• Give hands-on care to patients nearing the end of life, following state nursing rules .
• Follow the care plan after assessing the patient’s needs.
• Keep clear and timely records of the care provided.
• Teach and support patients and their families about the illness and care plan.
• Help coordinate care by communicating with the hospice team (like aides, chaplains , social workers).
• Manage pain and symptoms, checking how well treatments are working .
• Take part in training and quality improvement activities to grow professionally and improve care.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.
• Currently licensed as an LPN/LVN in the State.
• Comply with accepted professional standards and practice.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.
• Understand philosophy of hospice concept . Also understand needs of the terminally ill.
• Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.
• Demonstrate good verbal and written communication, and organization skills.
