Embrace Hospice in San Antonio, TX seeks an on-call RN to provide compassionate, 1:1 end-of-life care during after-hours. You will assess symptoms, administer medications as directed, and collaborate with the hospice team, while supporting patients and families in difficult times.

Requirements include RN licensure, a year of nursing experience (3+ preferred), and hospice exposure. A valid driver’s license and insured vehicle are necessary.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Must be a licensed driver with a vehicle insured and in good working order.

Responsabilités

• Provide 1:1 care to hospice patients during after-hours, on an on-call basis.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medications or adjust care per hospice protocols or orders.
• Communicate with the physician regarding changes in condition and orders.
• Offer guidance to patients and families about current condition and next steps.
• Pronounce death and provide family support if occurs during on-call.
• Complete timely documentation and communication.

Schedule:7 on 7 off Friday 5p-Mon 8am then M-Th 5pm-8am

As a On-Call RN You will:

• (CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis

• (CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis

• Assess and manage urgent symptoms quickly and compassionately

• Administer medication or adjust care based on hospice protocols or provider orders

• Communicate with the physician regarding changes in condition and obtain orders as necessary

• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps

• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team

• Complete timely documentation and communication

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation

• Knowledge of the hospice philosophy of care

• Commitment to clinical and documentation excellence

• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
