Embrace Hospice in San Antonio, TX is seeking a full-time RN on-call to provide compassionate, 1:1 end-of-life care during after-hours. You will respond to hospice needs in patients' homes or facilities, manage symptoms, administer or adjust medications, and coordinate with the physician.

The role offers a supportive team, flexible on-call schedule, mileage reimbursement, and a comprehensive benefits package including health/dental/vision, HSA/FSA, 401(k) match, PTO and Tuition reimbursement.

Formación

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsabilidades

• Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medication or adjust care based on hospice protocols or provider orders.
• Communicate with the physician regarding changes in condition and obtain orders as necessary.
• Offer guidance and support to the patient and family in understanding the current condition and next steps.
• If a death occurs during on-call hours, pronouncing death, providing support to the family, and notifying the hospice team.
• Complete timely documentation and communication.

Coverage area: San Antonio, Kerrville, New Braunfels

Schedule: 7 on 7 off Friday 5p-Mon 8am then M-Th 5pm-8am

As a On-Call RN You will:

• (CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• (CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• Assess and manage urgent symptoms quickly and compassionately
• Administer medication or adjust care based on hospice protocols or provider orders
• Communicate with the physician regarding changes in condition and obtain orders as necessary
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps
• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team
• Complete timely documentation and communication
