Job summary

Embrace Hospice in San Antonio, TX is seeking a compassionate Hospice RN Case Manager to deliver patient-centered care in a home-based setting. You will assess needs, create and implement care plans, and partner with families to ensure comfort and dignity at end of life.

The role emphasizes independent practice with team support, on-call coverage, and opportunities for professional growth in a supported environment.

Qualifications

• Graduate of an accredited nursing program with TX licensure.
• At least 1 year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Must have a valid drivers license and insured vehicle.
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients
• Create hospice care plans aligned with patients wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patients family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

As a RN Case Manager You will:

Qualifications

• Knowledge of the hospice philosophy of care

• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
