Embrace Hospice in San Antonio, TX is seeking a Registered Nurse Case Manager to join our hospice team. The role focuses on providing expert, patient-centered care at a critical time, coordinating plans and supporting families in a compassionate environment.

The RN will assess needs, create care plans, administer treatments, manage pain and symptoms, educate families, and collaborate with an interdisciplinary team. On-call rotation may be required.

Qualifikationen

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
