Embrace Hospice in San Antonio, TX is seeking an On-Call Registered Nurse to provide hands-on hospice care during after-hours. You will assess urgent symptoms, administer medications per orders, and communicate changes with the physician to ensure compassionate, high-quality end-of-life care for patients in their homes or facilities.

The ideal candidate holds an active Texas RN license with 1 year of nursing experience (3+ years preferred) and a valid driver’s license, enabling reliable home

Qualifications

• Graduate from an accredited nursing program with active Texas RN license.
• Minimum 1 year nursing experience; 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must own a valid driver’s license and reliable transportation.

Responsibilities

• Provide hands-on hospice care during after-hours on-call.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medications or adjust care per orders.
• Communicate changes in condition with the physician.
• Offer guidance to patient and family about current condition and next steps.
• Pronounce death and support families if death occurs during on-call, contact funeral home as needed.
• Complete timely documentation and communications.

Coverage area: San Antonio, Kerrville, New Braunfels

Schedule: 7 on 7 off Friday 5p-Mon 8am then M-Th 5pm-8am

As a On-Call RN You will:

• ( CHOOSE ONE: If office HA S a Remote Triage team ) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• ( CHOOSE ONE: If office DOES NOT have Remote Triage ) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• Assess and manage urgent symptoms quickly and compassionately
• Administer medication or adjust care based on hospice protocols or provider orders
• Communicate with the physician regarding changes in condition and obtain orders as necessary
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps
• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team
• Complete timely documentation and communication

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required , 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
