Job summary

Embrace Hospice in San Antonio, TX seeks a compassionate Licensed Practical Nurse to join our team. The LPN provides 1:1 patient care, follows care plans, and supports families at end of life as part of a dedicated hospice team.

Schedule is Monday–Friday, 8am–5pm, with benefits including comprehensive health coverage, 401(k) with company match, PTO, mileage reimbursement, and tuition discounts. Embrace Hospice is an affiliate of Adoration Health.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience. Hospice or medical/surgical preferred.
• Licensed LPN/LVN in the State.
• Comply with professional standards and practice.
• Licensed driver with insured vehicle.
• Current CPR certification as required.
• Understand hospice philosophy and needs of the terminally ill.
• Use knowledge and experience to make decisions and solve problems.
• Strong verbal and written communication and organization skills.

Responsibilities

• Give hands-on care to patients nearing the end of life, following state nursing rules.
• Follow the care plan after assessing the patient's needs.
• Keep clear and timely records of the care provided.
• Teach and support patients and their families about the illness and care plan.
• Help coordinate care by communicating with the hospice team.
• Manage pain and symptoms, checking treatment effectiveness.
• Participate in training and quality improvement to grow professionally.

As a Hospice LPN You will:

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.


• Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.


• Currently licensed as an LPN/LVN in the State.


• Comply with accepted professional standards and practice.


• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.


• Possess and maintain current CPR certification in states where required.


• Understand philosophy of hospice concept. Also understand needs of the terminally ill.


• Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.


• Demonstrate good verbal and written communication, and organization skills.
