Job summary

Accurate Home Care in Woodbury, MN is seeking a Licensed Practical Nurse (LPN) to provide in‑home patient care, perform assessments, administer medications, and coordinate with physicians and the care team to support recovery at home.

Qualifications

• Valid Minnesota LPN license required.

Responsibilities

• Implement the nursing process: assessment, diagnosis, planning, implementation, evaluation.
• Communicate with physician and multidisciplinary care team about patient health status.
• Provide care including tracheostomy/ventilators, feeding tubes, wound care and seizure management.
