Job summary

Accurate Home Care is seeking a Private Duty Nurse (LPN) to provide one-on-one nursing care in the patient’s home for adults and children. Join a team focused on accountability, compassion, integrity and teamwork in a supporting, home-based setting.

The role emphasizes the nursing process, collaboration with physicians, and delivering a broad range of care, including respiratory support and wound management, in a home environment.

Qualifications

• Active Minnesota LPN license required.
• Fluent in English (reading/writing/speaking).
• Knowledge of home health care scope of services.
• BLS CPR certification required (AHA or Red Cross).
• Ability to lift and transfer up to 50 lbs.

Responsibilities

• Implement the nursing process: assessment, diagnoses, planning, implementation, evaluation within scope of practice.
• Communicate with physician and other members of the multidisciplinary patient care team about health status.
• Provide care ranging from tracheostomy/ventilators to feeding tubes, wound care and seizure management.

• Basic Life Support (BLS) CPR course for healthcare providers through American Heart Association or Red Cross renewed at recommended frequencies.
• Must be able to lift, turn, position, and transfer up to 50lbs.
