Job summary

Accurate Home Care is seeking a Private Duty Nurse (LPN) to deliver in-home care for adults and children in Elk River, MN. You will perform nursing assessments, develop plans, and work with a multidisciplinary team to support patients at home.

We offer 8–12 hour shifts, a starting wage of $27/hour, benefits including health insurance reimbursement, 401k, CEUs, PTO, on‑demand pay, and a referral bonus up to $3,000. Our culture centers accountability, compassion, integrity and teamwork.

Qualifications

• LPN license valid in Minnesota.
• English fluency in reading/writing/speaking.
• BLS CPR certification current.
• Experience in home-based care is preferred.

Responsibilities

• Perform nursing assessment, planning, implementation, and evaluation.
• Coordinate with physician and care team for patient status.
• Provide adult and pediatric nursing care in the patient’s home.

What will I be doing?

• Implementing the nursing process: assessment, diagnosing, planning, implementing, evaluating based on scope of practice.
• Communicate with physician and other members of multidisciplinary patient care team regarding the health status of patient.
• Provide a variety of cares ranging from tracheostomy and ventilators, to feeding tubes, wound care, and seizure management.

• Ability to read, write and speak fluently in English.
• Education and experience in the scope of services offered by the agency.
• Basic Life Support (BLS) CPR course for healthcare providers through American Heart Association or Red Cross renewed at recommended frequencies.
• Must be able to lift, turn, position, and transfer up to 50lbs.
