Job summary

Accurate Home Care in Minnesota seeks a Licensed Practical Nurse (LPN) to join our team and support patients at home. You will apply the nursing process, assess health status, and collaborate with the care team to plan and implement care plans.

Ideal candidates hold Minnesota LPN licensure, fluent English, and BLS CPR certification. The role includes lifting up to 50 lbs and requires compassionate, accountable, and team‑oriented care aligned with our core values.

Qualifications

• Valid license as an LPN in Minnesota.
• Ability to read, write and speak fluently in English.
• Education and experience in the scope of services offered by the agency.
• Basic Life Support (BLS) CPR through AHA or Red Cross renewed at intervals.

Responsibilities

• Implementing the nursing process: assessment, diagnosing, planning, implementing, evaluating based on scope of practice.
• Communicate with physician and other members of multidisciplinary patient care team regarding the health status of patient.
• Provide a variety of cares ranging from tracheostomy and ventilators, to feeding tubes, wound care, and seizure management.
