Job summary

Complete Care, Inc. is seeking an LPN Licensed Practical Nurse to provide direct care for residents recently discharged from hospital or in long-term care. Full and part-time options are available; PRN not accepted.

Responsibilities include medication administration, wound care, monitoring, and collaboration with the care team. Qualifying candidates must be licensed in CT, pass background checks and screenings, and demonstrate leadership and clinical skills.

Qualifications

• Licensed to work within the State of Connecticut as a LPN Licensed Practical Nurse
• Able to pass background check, drug screen, professional references check & physical evaluation
• Professional & clinically skilled with leadership qualities

Responsibilities

• Administer medications: oral, topical, IV, and injectable medications
• Wound care and skin care including dressing changes and pain management
• Monitor health status and track changes
• Provide catheter/ostomy care and blood/gas sample collection
• Assist in executing individualized care plans
• Collaborate with physicians, therapists, social workers and others
• Educate residents and families about health status and care expectations
• Maintain detailed health records and care progress
• Oversee and delegate CNAs to ensure quality care

Responsibilities

• Administer Medications: Give oral, topical, IV, injectable medications
• Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings
• Monitoring: Observe physical and mental health status, track changes, analyze data

• Care Planning: Assist in executing individualized care
• Collaboration: Strategize care with physicians, therapists, social workers & others
• Family & Resident Education: Explain health status, changes and expectations for care
• Documentation: Keep a detailed record of health status, all care, progress & interactions
• Team Oversight: Oversee & delegate CNAs ensuring quality care & teamwork
