Job summary

Complete Care at Kimberly Hall North in Windsor, CT is seeking an LPN to deliver direct care for residents recently discharged or in long-term care. You will work with a multidisciplinary team to provide safe, compassionate care.

The role includes medication administration, wound care, health monitoring, and collaboration with physicians, therapists, social workers, and families to implement individualized care plans. Full and part-time options available. Daily pay options may be offered.

Qualifications

• Licensed to practice as LPN in Connecticut.
• Able to pass background check and screen tests.
• Professional and clinically skilled with leadership qualities.

Responsibilities

• Administer medications including oral, topical, IV, and injections.
• Perform wound care and prevention, manage pain, and apply dressings.
• Monitor health status and analyze data for changes.
• Provide catheter/ostomy care and collect specimens.
• Assist in executing individualized care plans.
• Collaborate with physicians, therapists, social workers, and others.
• Educate residents and families about health status and care expectations.
• Maintain detailed health records of care and progress.
• Oversee and delegate tasks to CNAs to ensure quality care.

Here’s What You’ll Do for Our Residents:

• Administer Medications: Give oral, topical, IV, injectable medications
• Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings
• Monitoring: Observe physical and mental health status- track changes & analyze data
• Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.
• Care Planning: Assist in executing individualized care
• Collaboration: Strategize care with physicians, therapists, social workers & others
• Family & Resident Education: Explain health status, changes and expectations for care
• Documentation: Keep a detailed record of health status, all care, progress & interactions
• Team Oversight: Oversee & delegate CNA’s ensuring quality care/teamwork
