Qualifications

• Pass background check, drug screen, and references.
• Clinical leadership qualities preferred.

Responsibilities

• Administer medications and therapies to residents.
• Provide wound care and dressing changes.
• Monitor patient status and report changes promptly.
• Perform catheter/ostomy care and collect specimens as needed.
• Assist in executing individualized care plans and educate families.
• Collaborate with physicians, therapists, social workers and other team members.
• Document health status, care provided and patient progress.
• Oversee and support CNAs to maintain quality care and teamwork.

Here's What You'll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Monitoring: Observe physical and mental health status- track changes & analyze data

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA's ensuring quality care/teamwork
