Qualifications

• Licensed to work in Connecticut as an LPN.
• Able to pass background check, drug screen, references check and physical evaluation.

Responsibilities

• Administer medications including oral, topical, IV and injectable forms.
• Perform wound care, prevention, manage pain and apply dressings.
• Monitor residents’ physical and mental health status and track changes.
• Provide catheter/ostomy care and collect specimens.
• Assist in executing individualized care plans.
• Collaborate with physicians, therapists, social workers and others.
• Educate residents and families about health status and care expectations.
• Maintain detailed health records and document care and interactions.
• Oversee and delegate CNA tasks to ensure quality care and teamwork.

Here’s What You’ll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing individualized care

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA’s ensuring quality care/teamwork
