Aventura at Shiloh Springs is hiring a Licensed Practical Nurse (LPN) to provide direct nursing care for residents according to care plans and physician orders. You will administer medications, monitor health status, and contribute to individualized care plans.

Qualifications

• Valid LPN license in the state of employment.
• Graduate of an accredited Licensed Practical Nursing program.
• CPR/BLS certification (required or able to obtain upon hire).
• At least 3 years of nursing experience (long-term care preferred).
• Ability to communicate effectively and compassionately with residents, families, and team members.

Responsibilities

• Provide direct nursing care to residents according to care plans and physicians’ orders.
• Administer medications and treatments accurately and in a timely manner.
• Monitor and document residents’ health status, reporting significant changes.
• Assist with resident assessments and development of individualized care plans.
• Supervise CNAs and other nursing assistants in delivering daily care.
• Ensure documentation is complete, accurate, and compliant with policies and regulations.
• Respond to emergencies and follow proper protocols.
• Support residents and families with health education and emotional support.
• Maintain infection control, safety, and HIPAA standards.
• Participate in interdisciplinary team meetings and in-service training.

Key Responsibilities:

• Assist with resident assessments and the development of individualized care plans.
• Supervise and direct CNAs and other nursing assistants in delivering daily care.
• Ensure all documentation is complete, accurate, and in compliance with facility policies and state/federal regulations.
• Respond to emergencies and follow proper protocols.
• Support residents and families with health education and emotional support.
• Maintain compliance with infection control, safety, and HIPAA standards.
• Participate in interdisciplinary team meetings, audits, and in-service training.

Qualifications:

• Valid LPN license in the state of employment
• Graduate of an accredited Licensed Practical Nursing program
• CPR/BLS certification (required or able to obtain upon hire)
• At least 3 years of nursing experience (long-term care preferred, but not required)
• Ability to communicate effectively and compassionately with residents, families, and team members
• Familiarity with electronic medical records (EMR) systems preferred
