Aventura at Carriage Inn is actively hiring a Licensed Practical Nurse (LPN) to provide dedicated, resident-centered care in a supportive setting. You will administer medications, monitor health, and collaborate with an interdisciplinary team to uphold quality of life for residents.

Qualifications

• Valid LPN license in the state of employment.
• Graduate of an accredited Licensed Practical Nursing program.
• CPR/BLS certification (required or able to obtain upon hire).
• At least 1 year of nursing experience (long-term care preferred).
• Strong communication and compassion with residents, families, and team members.

Responsibilities

• Provide direct nursing care to residents according to care plans and orders.
• Administer medications and treatments accurately and timely.
• Monitor and document health status and report changes to RN/physician.
• Assist with assessments and development of individualized care plans.
• Supervise CNAs and other nursing staff in daily care.
• Ensure documentation is complete and compliant with regulations.
• Respond to emergencies and follow proper protocols.
• Support residents and families with health education and emotional support.
• Maintain infection control, safety, and HIPAA standards.
• Participate in team meetings, audits, and training.

Key Responsibilities:

• Provide direct nursing care to residents according to care plans and physicians' orders.
• Administer medications and treatments accurately and in a timely manner.
• Monitor and document residents' health status, reporting any significant changes to the RN or physician.
• Assist with resident assessments and the development of individualized care plans.
• Supervise and direct CNAs and other nursing assistants in delivering daily care.
• Ensure all documentation is complete, accurate, and in compliance with facility policies and state/federal regulations.
• Respond to emergencies and follow proper protocols.
• Support residents and families with health education and emotional support.
• Maintain compliance with infection control, safety, and HIPAA standards.
• Participate in interdisciplinary team meetings, audits, and in-service training.

Qualifications:

• Valid LPN license in the state of employment
