Complete Care Management in Maryland is seeking an LPN Licensed Practical Nurse for direct care of residents recently discharged from the hospital or in long-term care. You will work closely with a multidisciplinary team to deliver high-quality care.

Responsibilities include administering medications, wound care, monitoring health status, catheter/ostomy care, and collaborating with physicians and therapists. Competitive wages with shift differentials, daily pay option, and strong benefits await.

Qualifications

• Must be licensed to work in Maryland as an LPN.
• Pass background check, drug screen, references, and physical evaluation.
• Professional and clinically skilled with leadership qualities.

• Administer medications (oral, topical, IV, injectable).
• Wound care and dressing management.
• Monitor physical and mental health status; track changes and analyze data.
• Provide catheter/ostomy care and specimen collection.
• Assist in executing individualized care plans.
• Collaborate with physicians, therapists, social workers and others.
• Educate residents and families about health status and care expectations.
• Maintain detailed health status documentation and care progress.
• Oversee CNAs to ensure quality care and teamwork.

Here’s What You’ll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Monitoring: Observe physical and mental health status- track changes & analyze data

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing the individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA’s ensuring quality care/teamwork
