Complete Care Management in Maryland is seeking a qualified LPN Licensed Practical Nurse to provide direct care to residents, administering meds, wound care, and monitoring health status.

Qualifications

• Licensed to work within the State of Maryland as a LPN.
• Able to pass background check, drug screen, references checks & physical evaluation.
• Professional & clinically skilled with leadership qualities.

Responsibilities

• Administer medications including oral, topical, IV and injectable meds.
• Perform wound care and prevention, manage pain and apply dressings.
• Monitor health status and analyze data to track changes.
• Provide catheter/ostomy care and collect specimens as needed.
• Assist in executing individualized care plans.
• Collaborate with physicians, therapists, social workers and others.
• Educate residents and families about health status and care expectations.
• Maintain detailed records of health status, care provided and progress.
• Oversee and delegate CNA tasks to ensure quality care and teamwork.

Here’s What You’ll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Monitoring: Observe physical and mental health status- track changes & analyze data

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA’s ensuring quality care/teamwork
