Job summary

Complete Care at Middlebury is seeking a part-time LPN Licensed Practical Nurse to join our direct care team serving residents recently discharged from hospital or requiring long-term care. This position offers 2nd shift options and PRN with experience, plus weekend differential pay.

You will administer medications, provide wound care, monitor health status and collaborate with physicians, therapists, and family to ensure quality care.

Qualifications

• Must be licensed LPN in this state.
• Professional & clinically skilled with leadership qualities.

Responsibilities

• Administer medications: oral, topical, IV, injectable meds.
• Wound care: dressing, prevention, pain management.
• Monitor health status and analyze data.
• Provide catheter/ostomy care and specimen collection.
• Assist in executing individualized care plans.
• Collaborate with physicians, therapists, social workers & others.
• Educate residents and families about health status and care expectations.
• Maintain detailed health records and care progress.
• Oversee and delegate CNA activities to ensure quality care.

Here’s What You’ll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Monitoring: Observe physical and mental health status- track changes & analyze data

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA's ensuring quality care/teamwork
