Job summary

Complete Care Management is seeking a dedicated LPN Licensed Practical Nurse to join our direct care team in West Virginia. You will administer medications, monitor residents, and collaborate with a multidisciplinary team to ensure high-quality care for patients recently discharged or in long-term care.

Responsibilities include wound care, clinical procedures, care planning, education for families, and meticulous documentation.

Qualifications

• Licensed to work within West Virginia as an LPN.
• Able to pass background check, drug screen, references, and physical.
• Professional and clinically skilled with leadership qualities.

Responsibilities

• Administer Medications: oral, topical, IV, injectable meds.
• Wound/Skin Care: perform wound care and dressings.
• Monitoring: observe health status and track changes.
• Clinical Procedure: catheter/ostomy care and specimen collection.
• Care Planning: assist in executing individualized care plans.
• Collaboration: coordinate with physicians, therapists and social workers.
• Family & Resident Education: explain health status and expectations.
• Documentation: maintain detailed health and care records.
• Team Oversight: supervise and delegate to CNAs for quality care.

Here's What You'll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Monitoring: Observe physical and mental health status- track changes & analyze data

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA's ensuring quality care/teamwork
