Complete Care at Oak Ridge is hiring an LPN Licensed Practical Nurse to provide direct care for residents in our Charleston, WV facility. You’ll administer medications, perform wound care, monitor health status, and support care planning with a collaborative team.

Qualifications

• Licensed to work within the State of West Virginia as an LPN.
• Able to pass background check, drug screen, professional references check & physical evaluation.
• Professional & clinically skilled with leadership qualities.

Responsibilities

• Administer medications: oral, topical, IV, injectable.
• Wound care and dressings; manage pain.
• Monitor physical and mental health status; track changes.
• Provide catheter/ostomy care and collect specimens.
• Assist in executing individualized care plans.
• Collaborate with physicians, therapists, social workers and others.
• Educate residents and families on health status and care expectations.
• Maintain detailed health records and care interactions.
• Oversee and delegate CNA tasks to ensure quality care and teamwork.

Here’s What You’ll Do for Our Residents:

• Administer Medications: Give oral, topical, IV, injectable medications
• Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings
• Monitoring: Observe physical and mental health status- track changes & analyze data
• Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.
• Care Planning: Assist in executing individualized care
• Collaboration: Strategize care with physicians, therapists, social workers & others
• Family & Resident Education: Explain health status, changes and expectations for care
• Documentation: Keep a detailed record of health status, all care, progress & interactions
• Team Oversight: Oversee & delegate CNA’s ensuring quality care/teamwork
