Job summary

Decatur Wellness & Rehabilitation Center in Decatur, Tennessee is seeking a compassionate LPN for night shifts to join our care team. You will direct daily CNA activities, participate in admissions and transfers, and medicate residents per physician orders.

Responsibilities include initiating and updating care plans, reporting changes to the nurse supervisor, maintaining confidentiality, and performing duties as assigned by leadership.

Qualifications

• Current unrestricted LPN License for the state of Tennessee
• Current CPR/BLS Certification

Responsibilities

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess resident’s conditions, prepare, and administer medications per physician orders.
• Initiate, review, and update care plans.
• Report any changes in a resident’s condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.
• Perform other assigned duties given by clinical or administrative leadership.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.
