Decatur Wellness & Rehabilitation Center is seeking Night Shift LPNs for 6 PM to 6 AM in Decatur, Tennessee. The role involves directing CNAs, managing admissions and transfers, and administering medications under physician orders to ensure high-quality resident care.

Qualifications

• Current unrestricted LPN License for the state of Tennessee.
• Current CPR/BLS Certification.

Responsibilities

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess resident’s conditions, prepare, and administer medications per physician orders.
• Initiate, review, and update care plans.
• Manage the nursing care for all residents and delegate shift tasks to nurses.
• Supervise nurse aides as they complete their assignments.
• Observe residents’ conditions, oversee, and respond as necessary.
• Inform physicians of any changes in resident conditions.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

LPN Position Summary

As a LPN at Decatur Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:
