Job summary

Complete Care at Meriden is seeking an LPN Licensed Practical Nurse to provide direct care for residents recently discharged from hospital or in need of long-term care. Full-time, part-time, and PRN with experience options are available.

You will administer medications, perform wound care, monitor health status, and collaborate with physicians, therapists, social workers and families to deliver coordinated care. We emphasize education, documentation, and teamwork in a supportive environment.

• Active LPN license to practice in this state.
• Professional clinical skills with leadership qualities.

Here’s What You’ll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Monitoring: Observe physical and mental health status- track changes & analyze data

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA’s ensuring quality care/teamwork
