Job summary

A leading company in home health services is seeking an LPN Intake Coordinator to manage incoming referrals and coordinate care information. This role is vital in liaising between patients, physicians, and the Intake Team, ensuring effective communication and service delivery. Candidates should have current LPN licensure and experience in medical office procedures.

Job description

Summary

The LPN Intake Coordinator receives and analyzes incoming referral and order calls.

Pay Rate: $33 - $35 hourly, depending on experience

Responsibilities:

  1. Answer phones and appropriately triage calls to the Intake Team.
  2. Communicate patient/referral information effectively to the appropriate Intake staff and branches.
  3. Obtain demographic and clinical information from clients, physicians, and referral sources to compile referrals.
  4. Coordinate communication of referral information with the Intake Specialist RN.
  5. Maintain a call log for all incoming calls.
  6. Fax signed referrals to the appropriate offices, DME, private duty, etc.
  7. Verify new referring physician licenses on department referrals.
  8. Research coverage or pay sources for each referral, including verifying commercial policies and HIQA.
  9. Obtain authorizations for visits from insurance or Medicaid for EPSDT patients as appropriate, and update case managers on patient status and additional visit needs.
  10. Educate prospective patients, families, physicians, and hospitals about available home health services.
  11. Serve as a liaison between clinical and marketing staff.
  12. Participate in annual in-service training programs.
  13. Manage patient ping feeds in applicable markets.
  14. Disseminate patient ping data to the appropriate care managers, clinicians, or sales representatives.
  15. Perform other duties as assigned.

Education and Experience:

  • Current LPN licensure in the state of practice.
  • Valid Driver's License, vehicle insurance, and access to reliable transportation.
  • CPR Certification.
  • At least one (1) year of experience in medical office procedures or intake.
