Overview:

Sentara is hiring a full-time and remote LPN Intake Coordinator based out of Chesapeake, VA.

Schedule

9am-6pm or 8am-5pm M-F

Rotating weekends and holidays

Job Responsibilities:

• Evaluating and processing referrals by receiving and responding to incoming calls from referral sources or potential patients.
• Recording the outcome of calls and making follow-up calls as necessary.
• Entering referral information into the system, including clinical details and service orders.
• Developing and maintaining a working knowledge of all services and resources provided by Sentara and available within the community.
• Acting as a resource for patient and referral source issues.

Education

• High school graduate or equivalent

Certification/Licensure

• LPN

• Minimum 1 year of experience with clinical and customer service in a healthcare environment
