A Brief Overview

The Home Care Intake Coordinator is responsible for triaging all incoming communications in order to obtain referrals necessary for the initiation of Home Care services and their successful reimbursement. The intake process includes but not limited to the the verification of insurances, service location, ordered services and entering the referral data.


What you will do

  • The ability to comprehend spoken and written English, to communicate with clarity both verbally and in written English, work independently, good organizational skills, good documentation skills, ability to use a telephone and operate a computer.
  • Non-combative and consistent behavior, ability to appropriately interact with various patient populations, staff and providers.
  • Evaluates referrals for Home Care including insurance verification, service location, skilled care ordered and homebound status.
  • Incorporates the clinical history, physiological and psychosocial of patients into the evaluation of a referral to insure the appropriateness of the referral.
  • Serves as liaison between referral sources (i.e. Hospital/Rehab staff, Provider Offices) and/or patients/families regarding the status of patient referrals and patient discharges.
  • Consults with Lead Intake Coordinator or designee to verify a safe and coordinated discharge plan.
  • Interprets Home Care policies to patients, hospital staff, and referral sources
  • Respond to all incoming forms of communication (faxes, web portals, calls) and triages clinical calls appropriately. Make appropriate communication to obtain referral and/or provide information for alternative community resources.
  • Provides phone support to field staff including making patient calls for scheduling confirmations and changes
  • Participates in educational activities, interdepartmental meetings and committees
  • Responds to telephone inquiries regarding information about home care services, makes appropriate calls to obtain referral and/or provides information for alternative community services
  • Accepts and completes additional responsibilities as directed


Education Qualifications

  • Trade/Vocational School Graduate of an accredited LPN program required


Experience Qualifications

  • 1-3 years Home Care Experience required and
  • Intake Data Entry Experience preferred
