Overview:

Sentara is hiring a full-time and remote LPN Intake Coordinator based out of Chesapeake, VA.

Schedule

9am-6pm or 8am-5pm M-F, rotating weekends and holidays

Responsibilities:

• Evaluate and process referrals by receiving and responding to incoming calls from referral sources or potential patients.
• Record the outcome of calls and make follow-up calls as necessary.
• Enter referral information into the system, including clinical details and service orders.
• Develop and maintain knowledge of all services and resources provided by Sentara and available within the community.
• Act as a resource for patient and referral source issues.

Qualifications:

• High school diploma or equivalent
• Licensed Practical Nurse (LPN) certification/licensure
• Minimum of 1 year of experience in a healthcare environment with clinical and customer service roles
