Wellpath is seeking an Licensed Practical Nurse to deliver high-quality patient care under RN supervision in our multidisciplinary teams. You will participate in assessment, planning, and evaluation processes and assist with medications and nursing documentation.

The role emphasizes compassionate care for underserved populations, ongoing professional development, and a supportive work environment with a focus on safety and collaboration.

Qualifikationen

• Graduate from an accredited school of nursing.
• Minimum of one year clinic experience preferred.
• Have and maintain current licensure as an LPN/LVN in the state of employment.
• Must obtain and maintain CPR certification.

Aufgaben

• Under RN supervision, assess patients' physical, psychological, and social dimensions within Health Care Unit and housing units; contribute to individualized treatment planning and consult with RNs and staff as needed.
• Administer medications, including IV therapy if certified, and perform diagnostic tests per protocol.
• Assist in controlled substance counts at shift start; follow safety procedures and standard precautions; respond to codes or health emergencies as needed.
• Assist in medical or minor surgical procedures; implement nutrition and therapeutic diet plans; document nursing encounters using SOAP charting and communicate with staff, physicians, and others.
• Attend mandatory staff meetings and training sessions and adhere to policies to ensure high-quality patient care.
