Job summary

Wellpath, a provider of care within correctional and community health systems, is seeking a Licensed Practical Nurse (LPN) to contribute to patient care in our Medication-Assisted Treatment program. The role involves collaborating with RNs to support assessment, planning, and implementation of nursing care.

The LPN will administer medications, including IV therapy where certified, assist in therapies, and participate in shift handoffs and documentation.

Qualifications

• Education: Graduate from an accredited school of nursing.
• Experience: Prefer a minimum of one year clinic experience.

Responsibilities

• Under RN supervision, assesses patients across physical, psychological, and social dimensions in Health Care Unit and housing units, contributing to treatment planning.
• Implements treatment programs, administers medications (including IV therapy if certified), and performs diagnostic tests as directed.
• Assists in controlled substance counts at start of shift and responds to health emergencies per standard guidelines.
• Documents nursing encounters using SOAP charting and communicates with staff, physicians, and other personnel for continuity of care.
• Attends mandatory meetings and training to maintain high-quality patient care.
