The ideal candidate has Hospice nursing experience, strong organizational and communication skills, and proficiency with EMR and Microsoft Office.

Qualifications

• Minimum of Hospice nursing experience with knowledge of Continuous Care services.
• Experience in nurse staffing, scheduling, recruitment, or clinical coordination.
• Strong organization and ability to respond to urgent staffing needs.
• Proficiency with EMR and Microsoft Office; NJ LPN license required.

Responsibilities

• Coordinate and staff all Continuous Care cases across New Jersey.
• Recruit LPNs for Continuous Care assignments in collaboration with HR.
• Coordinate onboarding of staffing agencies to build a reliable nursing network.
• Communicate schedules, patient care requirements, and documentation expectations.
• Provide clinical reports to the interdisciplinary team regarding patient status.
• Review CC orders for accuracy and follow up with clinicians.
• Ensure CC notes are complete and submitted on time.
• Monitor length of stay and coordinate transitions to other levels of care.
• Review and verify agency invoices; resolve discrepancies.
• Maintain records related to staffing, recruitment, and utilization.
• Collaborate with leadership, HR, and billing to support quality patient care.
