Grace Healthcare Services is seeking an LPN to deliver hospice continuous care across New Jersey. The role focuses on identifying patient and family needs and implementing the plan of care under the supervision of the RN Case Manager.

You will provide direct bedside nursing, document care and interventions, administer medications, and coordinate with the care team. Travel within NJ is required.

Formación

• Licensed Practical Nurse (LPN) with NJ license.
• Minimum one year of clinical experience.
• Understanding of hospice philosophy and patient needs.
• Willingness to travel within the state of NJ.

Job Summary

The Hospice Continuous Care LPN is pivotal in identifying the physical, psychological, social and spiritual needs of the patient and family. The LPN works under the supervision and direction of the RN Case Manager and initiates the appropriate interventions in support of the patient and family. The LPN also follows a comprehensive and responsive plan of care.

Job Description

• This position does not have supervisory duties
• Provide direct skilled bedside nursing care to patients.
• Ensure and document care and interventions provided as well as appropriate use of medication, durable medical equipment and supplies.

Qualifications

• NJ Nurse License required
• Minimum one (1) year of clinical experience
• Understanding of Hospice philosophy and needs of terminally ill
• Strong problem solving and critical thinking skills
• Ability to travel within the state of NJ
