Job summary

Serenity Hospice Care is seeking a compassionate Licensed Practical Nurse (LPN) to join our team as a Full-Time Weekend Baylor providing overnight hospice continuous care across Burlington, Camden, and Mercer Counties in New Jersey. The role requires an active New Jersey LPN license and the ability to work all three overnight shifts.

Qualifications

• Current, active New Jersey LPN license.
• Strong clinical, communication, and documentation skills.
• Ability to work all three required overnight shifts (Fri 7:00 p.m.–Sat 8:00 a.m., Sat 7:00 p.m.–Sun 8:00 a.m., Sun 7:00 p.m.–Mon 8:00 a.m.).
• Reliable transportation.
• Hospice, palliative care, or long-term care experience preferred.

Responsibilities

• Provide nursing care according to the patient’s plan of care under RN supervision.
• Administer medications and treatments as ordered within the LPN scope.
• Monitor patient comfort, symptoms, and condition; report changes to supervising RN.
• Support patients and families with compassion, dignity, and respect.
• Complete accurate, timely clinical documentation.
• Communicate with the hospice clinical team to support continuity of care.
• Be willing to perform patient visits and wound care when Continuous Care cases are not needed.

About the Role:

Serenity Hospice Care is seeking a compassionate, dependable Licensed Practical Nurse (LPN) to join our team as a Full-Time Weekend Baylor providing overnight hospice continuous care. As a Hospice LPN Continuous Care, you will provide high-quality, patient-centered care to hospice patients requiring continuous care throughout Burlington, Camden, and Mercer Counties.

Responsibilities:

• Provide nursing care according to the patient's plan of care and under RN supervision

• Support patients and families with compassion, dignity, and respect

Requirements:
