Job summary

Serenity Hospice Care in Hamilton, NJ is seeking an organized LPN Continuous Care Manager to oversee staffing and coordination of hospice Continuous Care services across New Jersey. This full-time, in-office role runs Monday–Friday, 8:30 a.m.–4:30 p.m., based in our Hamilton office.

You will recruit LPNs, monitor staffing coverage, manage documentation, and coordinate with the interdisciplinary team to ensure high-quality patient care and efficient operations.

Qualifications

• LPN licensure in NJ is required.
• Experience in hospice nursing and Continuous Care services preferred.
• Experience in staffing, scheduling, recruitment, or clinical coordination.
• Strong organizational and communication skills; detail-oriented.

Responsibilities

• Coordinate and staff Continuous Care cases across New Jersey.
• Recruit LPNs for Continuous Care assignments.
• Coordinate onboarding of staffing agencies in NJ.
• Communicate schedules and patient care requirements to nurses.
• Provide clinical reports to the interdisciplinary team.
• Review and verify Continuous Care orders for accuracy.
• Ensure CC notes are complete and submitted on time.
• Monitor length of stay and transitions to other care levels.
• Verify agency invoices against rates and documentation.
• Maintain records on staffing, utilization, and documentation.
• Collaborate with leadership, HR, and billing for quality care.
