Job summary

Grace Healthcare Services is hiring a Continuous Care LPN to provide direct bedside nursing care to patients across Middlesex, Union, and Somerset Counties in New Jersey. You will work under the RN Case Manager to implement the plan of care, document interventions, and monitor the patient’s condition with compassion and clinical precision.

The role requires an active NJ LPN license, at least 1 year of clinical experience, and the ability to travel within the state.

Job Summary: The Hospice Continuous Care LPN is pivotal in identifying the physical, psychological, social and spiritual needs of the patient and family. The LPN works under the supervision and direction of the RN Case Manager and initiates the appropriate interventions in support of the patient and family. The LPN also follows a comprehensive and responsive plan of care.

Job Description:

This position does not have supervisory duties
