Job summary

OneWell Health Care is seeking a dedicated Licensed Practical Nurse (LPN) for home health in Jefferson Township, PA. This field-based role supports individuals with IDD across community and home settings, delivering skilled nursing and medication administration under supervision of an RN Case Manager.

The LPN will implement individualized care plans, monitor health status, and provide education to families while complying with PA DOH, DHS/ODP, Medicaid HCBS, HIPAA and nursing board regulations.

Qualifications

• Must hold an active Licensed Practical Nurse license in Pennsylvania.
• CPR and First Aid certification required.
• Minimum of 1 year of nursing experience; IDD/home health experience preferred.

Responsibilities

• Implement individualized care plans and administer medications.
• Perform wound, catheter, feeding tube management and vital sign monitoring.
• Monitor patient conditions and report changes to RN Case Manager; document promptly.
• Educate families, caregivers and new staff; participate in team meetings and quality reviews.
• Maintain infection control, safety practices and regulatory compliance.
