Job summary

OneWell Health Care is seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing services to individuals with IDD in home and community settings across Pennsylvania. You will work under RN supervision to implement care plans, administer medications, and support health, safety, and quality of life.

Qualifications

Completion of an accredited Practical Nursing program.
Current and active LPN license in Pennsylvania.
CPR and First Aid certification required.

Responsibilities

Implement individualized care plans.
Administer prescribed medications and treatments.
Perform wound care, catheter care, feeding tube management, vital signs.
Educate families and caregivers and train new staff.
Maintain accurate documentation and participate in case reviews.
