Job summary
OneWell Health Care is seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing services in home and community-based settings across Pennsylvania. You will work under an RN Case Manager to implement care plans, administer medications, and support individuals and families in maintaining health and quality of life.
Requirements include a PA LPN license, CPR/First Aid, and at least 1 year of nursing experience, with prior IDD/home health exposure preferred.
Qualifications
- Current and active Licensed Practical Nurse (LPN) license in Pennsylvania.
- CPR and First Aid certification (required).
- Completion of an accredited Practical Nursing program approved by the Pennsylvania State Board of Nursing.
- Minimum of one (1) year nursing experience.
- Experience supporting individuals with IDD, home health, or long-term care preferred.
Responsibilities
- Administer prescribed medications and treatments.
- Implement individualized care plans under RN supervision.
- Perform clinical procedures including wound care and vital signs monitoring.
- Support families and caregivers with health education and training.
- Maintain documentation in EHR and comply with HIPAA and state regulations.
