Job summary

OneWell Health Care seeks a Licensed Practical Nurse (LPN) to provide skilled nursing services in home and community-based settings across Pennsylvania, working under the RN Case Manager. You will implement care plans, administer medications, perform procedures, and support health, safety, dignity, and quality of life for participants.

The role requires a PA LPN license and CPR/First Aid certification, with travel within PA.

Qualifications

• Completion of an accredited Practical Nursing program approved by the Pennsylvania State Board of Nursing.
• Minimum of one year nursing experience required.
• Experience supporting individuals with IDD, home health, or long-term care preferred.

Responsibilities

• Implement individualized care plans; administering prescribed medications and treatments.
• Perform clinical procedures including wound care, catheter care, feeding tube management, and vital sign monitoring.
• Document medications, treatments, assessments, and incidents in the EHR; ensure timely, accurate documentation.
