Job summary

OneWell Health Care in Blakeslee, PA seeks a dedicated Licensed Practical Nurse (LPN) for field-based home health care. You will implement care plans, administer medications, and support individuals with IDD in homes and community settings across Pennsylvania.

The role reports to the RN Case Manager and requires PA licensure and CPR/First Aid. Ideal candidates have at least 1 year of nursing experience, familiarity with electronic records, and the ability to work in various home environments

Qualifications

• Current and active Licensed Practical Nurse (LPN) license in Pennsylvania.
• CPR and First Aid certification (required).
• Experience in home health or with IDD/IDD-related settings preferred.
• Knowledge of Pennsylvania nursing regulations and Medicaid waiver environments.

Responsibilities

• Implement individualized care plans and administer prescribed medications.
• Perform clinical procedures including wound care and catheter care.
• Monitor vital signs and document changes; report to RN Case Manager.
• Educate families, caregivers, and staff on care plans and safety practices.
• Maintain infection control standards and accurate documentation.
• Participate in interdisciplinary meetings and quality improvement activities.
