Job summary

OneWell Health Care is seeking a dedicated Licensed Practical Nurse (LPN) in Phoenixville, PA. The LPN will provide skilled nursing services in home and community settings, under the supervision of an RN Case Manager, delivering personalized care to individuals with IDD and related needs.

The role requires PA LPN licensure, CPR/First Aid certification, and at least 1 year of nursing experience. Travel within PA, flexible schedule, and field-based work are involved.

Qualifications

• Must hold an active LPN license in Pennsylvania.
• CPR and First Aid certifications required.
• Minimum of 1 year nursing experience.
• Experience supporting individuals with IDD/home health preferred.

Responsibilities

• Implement individualized care plans.
• Administer prescribed medications and treatments.
• Perform clinical procedures (wound care, catheter care, feeding tube management).
• Monitor and document changes; report to RN Case Manager.
