Job summary

OneWell Health Care is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing services to individuals with IDD in home and community settings across Pennsylvania. You will work under the supervision of an RN Case Manager, administer medications, and implement individualized care plans.

The position offers a field-based arrangement with a flexible schedule, requiring a PA LPN license, CPR/First Aid, and a valid driver’s license.

Qualifications

Current and active LPN license in Pennsylvania.
CPR and First Aid certification required.
Completion of an accredited Practical Nursing program approved by the Pennsylvania State Board of Nursing.
Minimum of one year nursing experience.
Experience with IDD, home health, or long-term care preferred.
Valid driver’s license and reliable transportation.
Proficiency in electronic documentation systems.
Ability to use home-care medical equipment.
English communication skills.

Responsibilities

Implement individualized care plans and administer prescribed medications.
Perform nursing procedures including wound care, catheter care, feeding tube management, and vital signs.
Monitor, document, and report changes in condition to the RN Case Manager.
Educate families and caregivers; participate in interdisciplinary meetings.
Maintain infection control practices and compliance with regulations.
Assist with audits and clinical compliance reviews.
Complete training and professional development as required.
