Qualifications

• Active LPN license in Illinois is required.
• Basic Life Support (BLS) certification within 30 days of hire.
• One year or more of LPN experience preferred.

Responsibilities

• Passes all medications and hangs IV piggybacks per order.
• Charts assessments and care plans after RN initial assessment.
• Perform trach care, NG tube care and feedings as needed.
• Involve in dressing changes and ostomy care; coordinate with staff.
• Provide patient education and discharge instructions; communicate with physicians/APPs.
