You will work with an interdisciplinary team to implement individualized care plans and ensure quality outcomes.

The role includes medication administration, potential IV therapy, EMR documentation, and collaboration to maintain a safe, compassionate care environment for residents.

Qualifications

• LPN licensure in state of practice.
• CPR certification required.
• Specialized training or experience in gerontological nursing preferred.

Responsibilities

• Practice nursing care within the scope of licensure and state Nurse Practice Act.
• Collaborate with interdisciplinary team to define individualized plans of care.
• Pass and oversee medication ordering and administration; monitor IV therapy and medications.
• May initiate IV therapy if IV-certified.
• Maintain documentation in EMR and ensure accurate medical reports.
• Ensure safety and coordinate with resident services for continuity of care.
• Oversee certified staff and support regulatory compliance.
