Job summary

Lutheran Senior Services in Glen Carbon, IL is seeking an experienced Licensed Practical Nurse (LPN) to provide and coordinate care for residents, following the state Nurse Practice Act and licensure rules.

You will work with an interdisciplinary team to implement individualized plans, administer medications (IV therapy if certified), document in the EMR, monitor resident status, and ensure safety and regulatory compliance.

Qualifications

• LPN licensure in state of practice is required.
• CPR certification required.
• Specialized training or experience in gerontological nursing preferred.

Responsibilities

• Passes medication and oversees medication administration.
• Administers medications and treatments as ordered.
• May initiate IV therapy if IV-certified.
• Maintains EMR documentation and regulatory compliance.
• Monitors resident status and coordinates with care team.
• Ensures safety and quality of care within licensure scope.
• Coordinates with other services to ensure continuity of care.
