Lutheran Senior Services is seeking a Licensed Practical Nurse (LPN) to provide and coordinate care within the scope of practice for residents, adhering to state Nurse Practice Act and EverTrue policies.

The role involves collaboration with an interdisciplinary team, overseeing medication administration, and accurate EMR documentation. LPN licensure in the practicing state and CPR certification are required; gerontological nursing experience is preferred.

Qualifications

• LPN licensure in state of practice (required).
• CPR certification required.
• Specialized training or experience in gerontological nursing preferred.

Responsabilités

• Practice nursing within the scope of licensure and state Nurse Practice Act; maintain active license and comply with licensure rules.
• Collaborate with interdisciplinary team to define individualized care plans and ensure regulatory compliance.
• Pass and oversee medication administration, monitor IV fluids and medications as ordered.
• May initiate IV therapy if IV-certified.
• Maintain accurate documentation and care plans in the EMR; ensure safe, continuous resident care.
• Supervise certified staff and ensure adherence to EverTrue standards of care.
