Job summary

Community Care, Inc. Home Health in Monessen, PA is seeking a dedicated LPN to provide 1:1 private duty nursing for a 2-year-old male child with cardiac needs.

This full-time overnight role offers one-on-one care, a compassionate team, and the chance to make a real difference in a child's daily life. You will assess and support ADLs, administer meds, monitor vitals, and collaborate with families and the care team to deliver safe, child-centered care in a home setting.

Qualifications

• Current LPN (or RN) license in Pennsylvania.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience (home health preferred).
• CPR certification.

Responsibilities

• Assist with Activities of Daily Living (ADLs).
• Perform g/gj‑tube care/feedings/aspiration precautions.

What You'll Do

• Communicate clearly and compassionately with the care team and family
• Deliver exceptional, family‑centered pediatric care

What You'll Need

• Current LPN (or RN) license in the state of Pennsylvania
• Graduate of an accredited nursing program
• At least 6 months of nursing experience (home health preferred, but not required)
• CPR certification
• Reliable transportation and a valid driver's license
• A caring heart, strong independence, and a passion for pediatric care
