Job summary

Community Care, Inc. Home Health is seeking an LPN for one-on-one pediatric private duty care in Millvale, PA. The role is full-time, day shift, with a Monday–Friday schedule from 8:00 AM to 4:00 PM. You will provide 1:1 nursing for a 19-year-old male patient in a home setting.

Ideal candidates hold an LPN license in PA, have CPR certification, and at least 6 months of nursing experience. Home health experience is a plus. Weekend availability is noted as well.

Qualifications

• Current LPN license in Pennsylvania (PA).
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience (home health preferred).
• CPR certification required.

Responsibilities

• Assist with Activities of Daily Living (ADLs).
• Perform gj-tube care/feedings.
• Monitor vitals.
• Administer medications safely and accurately.
• Communicate clearly with the care team and family.
• Deliver family-centered pediatric care.
